Below are your travel forms available for you to download prior to your trip.

1. Registration Form: Instructions - Click on the tab titled "Registration Form" on the top of our website and fill out the following questions with your passport in hand. If traveling as a couple or sharing a room together, please make sure each traveler fills out their own registration form.  

Once you have completed all of the questions on the form, scroll down and click the "Submit" button. 

After submitting, you will see a green check, signaling a successful submission. We will now be able to process your information. 

2. Cuba Travel Affidavit: Instructions - Check off category 8 (Support for the Cuban People) then fill out the basic info below and carry this form with you on your travels.

3. Mandatory Pre-Departure form: Instructions - All travelers must complete an online form a maximum of 48 hours before their arrival. After travelers complete the form, they will be given a QR code that can be scanned on arrival. Make sure to click the letter "A" on the top right of the screen to change the language to English.
